This website is a must if you do not speak Japanese and you need to visit a non-English speaking hospital or doctor. It is a series of multilingual medical questionnaires for foreigners living in Japan.
Find your language (English is the orange one second from the top left), and then simply select your category corresponding to the hospital department you are visiting. You can print it and bring it along your appointment and both you and the doctor will understand because it is bilingual!
